REACHING POTENTIAL

About Us

The MNP Follow Up Program extends the work we started in the neonatal intensive care unit (NICU). Beyond what we do for premature and medically fragile babies during their first weeks and months of life, this clinic is an opportunity to stay in touch with the families we’ve gotten to know and whose babies we’ve been privileged to care for.

When babies “graduate” from the NICU, they go from an intense, highly monitored, literally wired-in environment to their own homes. The transition, while welcomed, can be scary for families. This Follow Up Program provides a place for parents to showcase their children’s achievements, confirm progress and receive additional support as needed.

As we are a new clinic, we will be open on Fridays from 8 am - 4 pm with the goal of adding days and expanding upon our services as we continue to build our multi-discipline team. Continuing our close relationships with pediatricians, we complement the work they are doing with high-risk infants and toddlers. In partnership with your child’s primary care provider, we are proud to be a part of your team.

Our Vision for the clinic - To deliver transitional care and support for NICU graduates and high-risk infants as they continue their wellness journey, so that they can reach their fullest potential.
